Job Description:
HCA Healthcare invites applications for a dedicated RN
Registered Nurse specializing in Medical Discharge to join our
expert healthcare team. This role requires managing the process of
discharging patients from our care while ensuring a high standard
of treatment continuity and liaison with other healthcare
professionals. The ideal candidate will coordinate patient
discharges, educate patients and their families, and work
collaboratively across teams to ensure a seamless transition to
home care or other facilities. Coordinate and manage all aspects of
patient discharges. Develop discharge plans in consultation with
patients, their families, and multidisciplinary healthcare teams.
Ensure all discharge instructions are clearly communicated and
understood by patients and caregivers. Follow up with patients as
necessary to ensure post-discharge care plans are being followed.
Maintain accurate patient records and reports. Provide training and
support to junior nursing staff. Strong interpersonal and
communication skills Proficiency in healthcare management systems
Ability to work effectively in a team Excellent organizational and
problem-solving skills Medical Insurance Dental Insurance Vision
Insurance Life Insurance 401k Current RN license CPR certified
Certification in Case Management (preferred but not required)
