We currently have over 35 wholly owned locations, 130 affiliated locations and 100+ providers that deliver professional, friendly, and quality care to their patients. We are currently the 5th largest value-based care platform in Florida. With continued significant growth on the horizon.

Location: Brooksville, FL
Schedule: Mon-Fri 8am - 5pm
Job Description:
Typical outpatient Primary Care services
Managed Care
Patient Population: Geriatrics Ages 65+
PPD: 12-16 PPD
EMR: eClinical Works
Credentialing: 60-90 Days
Background and Certifications required:
BC, BE, GP (All considered)
Must have unrestricted FL license
Compensation:
Base Salary: 230-240K
Quality Metrics Bonus: 15% of annual salary
