Weekend Nurse Care Coordinator

The Position: We're actively looking for a full-time Weekend Nurse Care Coordinator

  Responsibilities:
  
  • 
    
  • Friday, Saturday, Sunday 8:00 AM to 8:00 PM
    
  • Meets With Patient and Family to Discuss Appropriate Hospice Services
    
  • Maintains Professional and Clinical Knowledge
    
  • Effectively Communicates with Patient, Family and Physician to Facilitate Smooth Admission
    
  • Support Family Members
    
  • Collaborate with Attending Physician and Family Members
    
  • Coverage Area: West Pasco County
    
    Requirements:
    
    • 
      
    • Registered Nurse in The State of Florida
      
    • Must Be Vaccinated or Exemption
      
    • Hospice or Acute Care Experience (Hospital, Surgery, Home Health)
      
    • Must Be Efficient with Computers
