Job DescriptionRegistered Nurse – Hospice Case ManagerWe have recently engaged with a progressive, well-run, and financially stable Hospice organization in the Wesley Chapel area. They are a not-for-profit organization committed to patient satisfaction, clinical excellence, and quality care for each family member.The Company: Our client's mission is to make quality hospice care available to all persons, their families, and those affected by serious illness, death, and dying in their community; to advocate effectively for the patient's comfort, dignity, and choice; and to be recognized as the leading resource in clinical, ethical, and spiritual issues of dying and grief.The Position: We're actively looking for a Hospice Case Manager in the West Pasco County, New Port Richey, FL area.   • Monday – Friday 8:00 AM to 5:00 AM
  • 4-5 Patients Per shift - Backup On-Call Only
  • No Admissions
  • Provide High-Quality End-Of-Life Care
  • Support Family Members
  • Notify Necessary Parties of Patient Deaths
  • Collaborate with Attending Physician and Family MembersRequirements:
    • Registered Nurse in The State of Florida
    • Must Be Vaccinated or Exemption
    • Hospice or Acute Care Experience (Hospital, Surgery, Home Health)
