Management:--Supervises all direct patient/family care team
members, including nurses, Supportive Care, home health aides and
counselors.Responsible for oversight of and/or scheduling, payroll
and other administrative dutiesAssures that all initial assessments
and required reassessments are conducted by qualified team members
within required time frames, including documentation completion,
submission and /or synchronization.Oversees and supports the IDG in
development, implementation and review of the interdisciplinary
plan of care for each patient/family.Collaborates with Site
Leadership members to monitor, manage and mentor all staff in
patient care activities, productivity, and documentation per
related policies and protocols.Monitor staffing reports to ensure
sufficient personnel for delivery of patient/family services,
including on-call. Makes staffing recommendations to the Executive
Director as necessary.Participates in the selection and evaluation
of all direct patient/family care team members--Clinical Leadership
Support:--Ensures that all patient/family services comply with
federal, state(s) and local regulatory requirements and with
accepted professional standards.Ensures protocols are utilized to
guide operations and promote adherence to regulatory and internal
process requirements.Monitor expenses related to patient/family
services to ensure budgetary compliance.Serve as a resource person
and liaison for the Social Worker, Volunteer, Music Therapy and
Bereavement Departments.Develop, coach and serve as a resource and
liaison to the TD's, DSC's, DESD, SDS and Clinical
Team(s).Responsible for the daily operations in assigned office of
the Hospice.Participate in the evaluations of vendors and providers
affiliated with the hospice on a contractual basis. Provides
feedback to the Vice President, Quality Assurance & Performance
Improvement on the quality of services provided by these
vendors/agencies.Holds Team Meetings as needed (at least Monthly)
for education of the Team Director, Clinical Teams. This meeting
will include reviewing current caseloads, ongoing education
regarding medications, levels of care, case study scenarios,
educational updates and other areas as appropriate.Review all new
admissions daily and assist in meeting documentation requirements
per protocols.--Education:--Participates and promotes Back to
Bedside efforts in reinforcing Case Management skills, knowledge
and attitude per protocol for all disciplinesProvides external
education / in-services to community partnersCollaborates with
Executive Director, Team Director(s), SDS/Site educator in
assisting with education planning and staff education
activitiesParticipates in the orientation and ongoing education of
staff and volunteers.--Interdisciplinary Group:--Attend the
interdisciplinary group meeting on a regular basis and serves as a
liaison with the Medical Director and clinical staff.Oversees IDG
Meeting Preparation: May prepares patient / care quality data
analysis (may include: AE's, triage trending, med costs, red/yellow
patients, OA, Advance Directive issues etc.) to provide timely
staff support and feedback as necessary during and/or after
meetingMay review above prepared data analysis with Associate TD
prior to IDG MeetingMay facilitate IDG team meetings:Engages and
encourages all IDG participationPrepares and follow up on
identified patient care concerns (see prep above)May coordinate
with TA to ensure regulatory documentation requirements addressed
as needed.Ongoing IDG Support: Utilizes proactive management
strategies to assist staff, patients and their families, which may
include:May triages incoming calls during business hoursProvides
caller support and assistancePerforms quality callsDocuments all
above per protocolOversees / facilitates CMR timely follow up and
provides staff supportWill assume responsibility for direct patient
care and administrator on call duties as
required.--Quality:Investigates, follow up with staff / process
issues, and documents outcomes per protocol regarding:Adverse
EventsTriage Trending resultsQuality of Care CallsCircle of Care
CallsDeyta / CAHPs and other quality measures /
outcomesParticipates in or leads quality improvement activities,
including data collection and aggregation as able--Participate in
ongoing quality assurance activities related to patient and family
services, including Quality Assurance & Performance Improvement
(QAPI) studies and chart audits to ensure the quality of
patient/family services.----Communications:--Reinforces and
supports all internal / external communications:Facilitates
administrative process changes / messagingParticipates in the
development, implementation and evaluation of policies and
procedures for patient/family services.Ensures all
interdepartmental communication and exchange of data is accurate,
timely and effective.Provides proactive customer service
maintenanceCommunicates as needed with all members of the team, the
patient/family, the Medical Director, the Director, Clinical
Operations and others involved in the care of the patient to
facilitate optimal care of patients/families, especially during
transition situations.Participates on committees and special
projects and other duties as assigned.Ensure that all patient and
family services offered by the Hospice comply with federal,
state(s) and local regulatory requirements.May participate in the
hiring and evaluation process of clinical staff.When necessary,
assume responsibility for a patient/family that includes assessing,
QualificationsRegistered Nurse license in state(s) practicing with
strong clinical skills.--Minimum 1 year management/supervisory
experience in a hospice or home health agency or equivalent
education.--Minimum three years' experience in hospice, home
health, general medical surgical, oncology or patient population
specific (i.e.: geriatrics, pediatric, etc.) nursing.
